All policies are regularly reviewed and updated. Please visit this page to stay up to date. However, we will not use your existing information in a manner not previously disclosed, and you will be advised of opportunities to opt out of new use of your information.

If you wish to bill your insurance independently, the front desk has the necessary forms. Accepted forms of payment are Visa, MasterCard, checks and cash. We do not accept American Express. Payment and/or co-payments are expected at the time of service. Outstanding accounts that exceed 180 days will have carrying charges at the rate of 1.5% per month (18% per year) No new appointments will be scheduled until balance due is paid in full.
Our Cancellation and Missed Appointment Policy
To ensure the compassionate and comprehensive care
necessary to diagnose and treat ME/CFS and Fibromyalgia, appointment times range from 20 minutes to 2 hours. To continue this high level of patient-centered service, it is essential to arrive 15 minutes before your appointment time and give appropriate notice when canceling or rescheduling.
- A cancelled or missed appointment without 48 hours prior notice will be charged a late cancellation fee. This fee will be equal to 50% of the scheduled visit cost.
- The exception is new patient visits, which occupy a 90-120 min block on the schedule and require preparation time; this will require a $200 cancellation fee.
- Appointments are considered “missed” at 15 minutes past the scheduled appointment time
No new appointments will be scheduled until balance due is paid in full.
